ACGME Fellowship Program - Geriatrics - How to Apply

Applications for the fellowship program may be obtained by e-mail from ther Fellowship Coordinator, Marilyn Tapper or can be downloaded from the Fellowship Application link. Application forms are in PDF format and contain instructions for completion.

After we receive your completed application including three recently written letters of recommendation and other supporting documents your file will be reviewed. The Director will only contact considered applicants for interviews. Applicants must be eligible for certification in Internal Medicine by the American Board of Internal Medicine or in Family Practice by the American Board of Family Practice. Applicants must be licensed by the State of Illinois at the time they begin their fellowship.

Earlier application submission is encouraged to allow adequate time for scheduling interviews and processing applications.
